Clinical Presentation

The bullae of pemphigus vulgaris originate from normal-appearing skin, there is essentially no surrounding inflammation. The blisters are also extraordinarily fragile. Immunonuorescent direct study carried out in perilesional skin shows a pathognomonic pattern of IgG removal on a network-like pattern surrounding the epidermal cells. Complement components are sometimes present. More than 90% of patients also have specific circulating autoantibodies. These antibodies showed directly immunofluorescent study. The titer of these antibodies roughly corresponds to the severity of the disease. Thus, reducing the antibody titer can be used as an indication of response to therapy.

Atypical Presentation
